Cannulation of a bile duct with periampullary diverticulum using an anterior-viewing endoscope with a cap

A 70-year-old man was referred to our hospital because of elevated hepatobiliary enzymes. He admitted for endoscopic retrograde cholangiopancreatography (ERCP) abdominal echography showed dilatation the intrahepatic bile duct and common duct, contrast-enhanced CT thickening wall lower duct. The large papilla located near inferior border diverticulum could be imaged from front. However, supporting tissue weak retracted into diverticulum, making insertion difficult. After changing an anterior-viewing endoscope with a cap fixing cap, became possible, patient diagnosed cholangiocarcinoma. This method may effective in cases periampullary tissue.
